Mr. David S. Price passed away on Sunday, January 18, 2004, at the Parkland Health Center, South, in Farmington, at the age of 40 years, 1 month & 9 days. David was born in Virginia on December 9, 1963. SURVIVORS INCLUDE : His MOTHER, Anna Maria Melkowski, of Upland, California; His WIFE, Rebecca Sue Annis, of Farmington; 1 BROTHER: Darrell (& Diana) Easley, of Tulsa, Oklahoma. Visitation will be Tuesday, 6-8 p.m., at TAYLOR Funeral Service CHAPEL, Farmington. Private Services & Interment will follow at a later date. Memorials may be given in David's memory to the charity of your choice.
